You'll be fine. Just don't start taking cups instead of spoons of medicine!
Is one a tablespoon and the other a teaspoon? If they actually hold those amounts, the bigger one holds three times more than the small one. Depending on the actual medicine, it probably won't hurt... As for that medication you took 2W... You should be fine. The photo of the two spoons you showed did not seem to show very much variation in terms of the size of the area the medicine would have been dealt. Unless the picture of those spoons had been taken at an angle to make everyone think that.
Like 'mute had stated, even if the difference in size of both spoons is as great as the difference between a teaspoon and a tablespoon, there isn't too much risk involved. If it is a medication that makes you drowsy, you will just feel so, faster.
Also... "recommended dosage" is determined on the average hieght and weight of the average person. i don't know if that realy makes much sense.
