Nah, St. Elsewhere used a move close to the "its a dream" segment. It ended with the hospital being no more than a snow globe that a child with douns syndrome played with, and fantasized about.

Yeah. I don't think that is what's going on.

The whole brain thing... maybe... but less likely then the dream sequence. It's along the same lines...
